Enrollment forms are provided below. If you would prefer, you can receive enrollment forms by email by sending a request to office@faithchristianacademytucson.org. Be sure to include "Enrollment Form" and "New / Returning Student" in the Subject bar.

Completing Forms:
Forms can be completed by:
- Printing and completing by hand
- Downloading to your computer and completing in PDF. You will need to have "Adobe Acrobat Reader DC" on your computer to save and complete the forms. (download free HERE)
Submitting Forms:
Saved or scanned forms should be emailed directly to documents@faithchristianacademytucson.org.
