Enrollment Forms

Enrollment forms are provided below.  If you would prefer, you can receive enrollment forms by email or by sending a request to office@faithchristianacademytucson.org.  


Be sure to include "Enrollment Form" and "New / Returning Student" in the Subject bar.


Completing Forms:

Forms can be completed by:

  1. Printing and completing by hand
  2. Downloading to your computer and completing in PDF.  You will need to have "Adobe Acrobat Reader DC"  on your computer to save and complete the forms. (download free HERE)

Submitting Forms:

Saved or scanned forms should be emailed directly to office@faithchristianacademytucson.org.

Returning Student:

Before you Start

Before you begin, make sure you have the following documents available to send by email. 


ALL STUDENTS:

NEW STUDENTS

Alternate Pencil  Copy of Birth Certificate

Alternate Pencil  Unofficial school records (including attendance, report cards, and disciplinary reports)

Alternate Pencil  Any IEP/ISP or 504 paperwork
