The following documentation is needed to complete the Emergency and Health Record.


  • Updated Immunization Record - if child has had immunizations since last year. 
  • Immunization Waiver (personal, religious, or medical): To access forms click HERE
  • ADD/ADHD Diagnosis Documentation- if applicable
  • 504 Documentation- if applicable
  • Custody Documentation – if applicable

  • By last day of school $150
  • After last day of school $300


NOTE:  Your application will not be accepted without both payment of enrollment fee

and completed paperwork.  Final Enrollment Fee owed is dependent on the date of application completion (not submission).
